JEWEL'S CATCH ONE PARTY REQUEST FORM

We can accommodate you with a free party Sunday. A $50.00 non-refundable deposit, is required for Saturday parties, to be paid at least one week before the date of your event. (Your reservation will be confirmed upon receipt of your guest list.  This offer applies to birthday parties only ~ No exceptions). In addition, we request that you treat this submission as a business document, and submit it with your first and last real name. 

Once we confirm your date, we will provide you with a free setup of table(s) and chairs. You can provide your own decorations, cake, and arrange for food from our in-house kitchen. You cannot bring in outside food. All decorations must be in place, no later than 6:00PM on the date of your event. You will not be allowed to decorate during regular club hours. That's it! Just make sure that all of your guests arrive before 11PM for free entry, and present their ID to the cashier, so they can be directed to your table(s). We look forward to assisting you with your celebration!
